Welcome to the new ThyCa online forms web site page. We continue to create a number of online forms to help you stay in better contact with us, as well as to manage your online profile with more to come in the future. Maximize the value of your connection to ThyCa by using this section to get the information you need ensuring the best benefit to you.
Additional features will be added over time, so be sure to check back to our new online web site page frequently to see those developments as well.
We are pleased to tell you that we have the following online sign-up forms, on www.thyca.org, for
1. Guestbook (including free newsletter subscriptions plus the options of requesting thyroid cancer materials from ThyCa and offering to volunteer for ThyCa)
2. Spring Workshop registration, and Conference Registration
3. Memberships and Renewals, Spirit Merchandise
4. Donations, to include individual fundraising events, and more new online forms
As always, no identifying information will be shared, sold, or loaned to any individual or organization outside of ThyCa.
If you choose to use one of the new forms, you will be prompted to fill out a "Login Screen." The Login Screen will ask for your Login and your Password, because you are already on our e-mail list due to prior interaction with ThyCa (such as signing up on the Guestbook or registering for a Workshop or Conference). If you forget either your Login or your Password, you can click Forgot Login or Password.
After you first use this system assigned password, we encourage you to change your randomly assigned password at your earliest convenience, to a password of your choosing.
If you wish to review and update your profile information, please select the profile menu option on the menu, or from the home page right column, and use your System Assigned Login, as well as the Password that will be sent separately. We invite you to login and review the information in your profile. Please make any corrections to your information. Thank you for your help.
